Help a pedestrian trying to understand how to wire up a BMS?

Here is what my pack looks like now.

Here is the diagram my board came with.

That's how I see it but I am still unsure. I would like to be 100% positive I am doing this proper. Please forgive my ignorance.

What's breaking my confidence is that the yellow leads on the diagram terminate at a single point despite there being two leads.

Am I to wire both yellow leads into an xt90?

Where am I getting confused?

Will I need to be running two connectors from this pack? One for the charger and one for the controller?

Is that why there are two yellow leads from P-?

Will a single xt90 serve as the charger connector and the controller connection as well? That doesn't seem right to me.

The Black leads seem to both run to the negative terminal on the pack.

I will be doing more homework but any leads are greatly appreciated. I am not going any further until I am confident.
 
Both yellow go to the same place. Your bms has two wires, to carry more load. Instead of one wire, you have two. It will need a Y splice, then connect to the discharge connector with as large a wire as the connector can handle. For 40 amps,, 12g should be big enough wire.

I am not getting the 46V from the connector that I get when I measure directly to the terminals.

This is wrong, right?
 
It looks like most positive cell triggered LVC as it has only 3.4V and LVC has been set to too high voltage...
The BMS still can be good enough...

As I see, other cells are also almost flat as their voltage is between 3.5 and 3.6V...

Try to charge the pack (it is almost empty) and check cell voltages during charging.
Don't allow any cell to go above 4.25V while charging the battery.
When happens that any cell goes above 4.25V, stop charging and leave battery to rest for a hour or two and check if voltage of that cell drop.
