Yes I did, I've adjusted the shifter many times, and as I mentioned in my original post, it's not really a problem for me because it never happens when I start off in a lower gear. I just asked out of curiosity.

Next I would check if the wheel or the bike is flexing toward the chain side under load enough for the chain to become out of alignament.
Check your derallieur bolts and deralieur hanger bolts (they go through the frame, usually from the inner side of the wheel).
Check if you suspension has side play or if your wheel has side to side play.

I had the exact problem and the hub axle wasn't tight enough (it was tight but I had to make it very tight).
